How to fill and sign PDF forms:

1. Download and install Adobe Acrobat Reader DC for free if you do not already have it. Click here for instructions.

2. After downloading a document from the list on this page, save it on your desktop as an Adobe Acrobat Document.

3. Open the form in Acrobat or Reader.

4. Choose Tools > Fill & Sign or choose Fill & Sign (in Adobe Reader) from the right pane.

5. Fill & Sign Tools will allow you to do the following:

• Add Text
• Add Checkmark
• Place Initials
• Place Signature
