You may download the form, fill it out and return it to us via email, fax or post. Before submitting, please read our complete Guidelines for Funding. Please note that the Project Proposal Form is NOT a Grant Application; it is a REQUEST for a Grant Application. If the project proposal meets our funding criteria, a Grant Application will be sent to you by post.
IMPORTANT - For projects outside the USA only:
Proposal must include a Letter of Support from the General, Regional, Provincial or Delegate Superior.
Letter of Support must be signed, on letterhead and have the official stamp of the congregation.
